USD 378 and Riley County Health Department are providing a Flu Shot Clinic on Tuesday, Oct. 4th, 2022 beginning at 8:30am at the Grade School. (When they are finished at the Grade School, they will then travel to the High School for those students/staff) There are no fees, however a copy of insurance card (front & back) is requested. Flu shots will be available for 1st - 12th grade students and all faculty/staff.
COVID vaccine boosters for ages 12 & up will also be available. RCHD will only have Pfizer available.
In order to receive a flu shot and/or COVID vaccine booster, each person must have:
- completed/signed registration form
- completed/signed consent form
- copy (front and back) of their insurance card
- copy (front and back) of their COVID vaccination card (if receiving a COVID booster)
You only need to complete 1 registration/consent form for each person even if they are receiving both a flu shot and COVID booster.
You may visit the following link to sign up for the clinic. Using this link you will also be able to upload scanned images of your insurance/COVID cards to bypass bringing a physical copy.

Even if you sign up using the link above, you still MUST turn in a completed/signed registration & consent form for each person. If these forms are not completed/turned in, the person will not be able to receive any vaccinations.
