USD 378 Riley County Schools has decided to take a proactive role in helping keep students, staff, and the families in our community healthy this flu season.
The registration link will expire at the end of the day on Tuesday, October 1st. People can continue to register by filling out the attached “All-in-One Influenza Registration Form.” If unable to register on-line, click the following link https://www.usd378.org/o/riley-county-usd-378/browse/81892 to access the form and information, but please contact your school nurse prior to October 3.
**Student Flu Clinic:
Date/Time: Thursday, October 3th, 2019
Grade School: 9:00-11:00 am
High School: 1:00-3:00 pm
Consent Form Deadline: Monday, October 1nd by end of day
We will need the following completed prior to your clinic:
- Influenza Consent Form; completed for each person receiving the flu vaccine, PRIOR to the clinic.
